CASE MANAGER

Beach House Center for Recovery is a well-established treatment facility in Palm Beach County that offers a full continuum of care to adults struggling with addiction and co-occurring mental health disorders. Our goal is to give patients the freedom they have been waiting for by placing an emphasis on the healing power of love and connection.


We are currently looking for a Case Manager, with excellent communication skills, to add to our clinical team. The case manager may work at either of our campuses in Juno Beach and North Palm Beach .

Responsibilities Include:

• Completes comprehensive psycho-social and needs assessments
• Serves as the primary point-of-contact for Clinical Staff for discharges
• Maintains active files and networks for community referrals, specifically related to aftercare programs available to clients upon discharge

Qualifications Include:

• 1+ year of Case Management experience
• Bachelor's Degree in Psychology, Social Work, or related field preferred
• Proficient with Microsoft Office Suite and an EMR Software
• Strong communication skills (written and verbal)

Salary is commensurate with experience

Schedule:

• 8 hour shift
• Weekend availability

Ability to commute/relocate:

• Juno Beach, FL 33408: Reliably commute or planning to relocate before starting work (Required)
