LVN Case Manager - Care Coordination & Community Health

AltaMed is seeking an LVN Case Manager in Los Angeles to provide daily care coordination, case management, coaching and intervention for patients with chronic diseases. You will work within an interdisciplinary team including social work, mental health, and in-home services to coordinate care across inpatient and outpatient settings. Requirements include graduation from an accredited LVN program, active LVN license, and 3+ years in public health or related fields with 2+ years in managed care. #J-18808-Ljbffr AltaMed
