Case Manager-LPN

Job Description The primary purpose of this position is to be a member of a patient’s care team in order to assist in planning, developing and coordinating patient care in accordance with applicable federal, state and local standards, guidelines, and regulations to assure that the highest degree of quality care can be maintained at all times. Responsibilities Meets with administration, medical and nursing staff, as well as other related departments in planning services‑to include patients and family Partners with the patient, family members, patient’s care team and Social , health and community agencies to insure a smooth transitions upon discharge from the Center Ensures that all documentation is informative, descriptive and follows the Center’s policy and procedures. Assists with the admission and discharge process.
