The University of Southern California Keck School of Medicine Street Medicine program seeks a Licensed Vocational Nurse Case Manager to support the Enhanced Care Management initiative in Los Angeles. This full-time, in-person role collaborates with a multidisciplinary team to provide case management, clinical care, and linkage to community resources for unsheltered patients, with a schedule Monday to Friday, 8 a.m. to 4 p.m. #J-18808-Ljbffr University of Southern California
