Registered Nurse Case Manager (RN)

This position offers a $15,000 sign on bonus. Schedule: Mon - Fri, 8:15am - 5:00pm. Territory: Zanesville, Caldwell, Duncan Falls, Kimbolton, New Concord, Quaker City areas. Company: Genesis Home Care. How You'll Make a Difference As a Registered Nurse Case Manager (RNCM) you will help patients spend enhanced quality time with loved ones in their preferred environment. You will provide one on one patient care, work at the top of your license, and coordinate care as directed by an attending physician. You will be part of an interdisciplinary team that focuses on compassionate quality care and positive outcomes. How You'll Work Assume ultimate accountability and leadership for patient assessment and care planning. Case manage and provide clinical care under physician direction, educate patients and families, ensure patient safety, and work autonomously while efficiently managing your time. Major Areas of Responsibility Plan of Care: Observe and monitor patient conditions; perform OASIS assessments; develop and adjust individualized care plans. Minimize Patient Risk: Administer medication as prescribed and reduce rehospitalizations by prioritizing high‑risk patient visits. Supervision: Oversee care provided by nurse aides and LPNs. Continuity of Care: Manage multidisciplinary care and promote continuity with admissions, transfers, and discharges. Family Educator/Advocate: Educate patients and families on disease processes using teach‑back methods. Policies: Complete clinical documentation following agency protocol and Medicare/Federal guidelines; collaborate with the interdisciplinary team and physicians. Rules & Regulations: Follow agency policies, procedures, regulatory changes, and communicate schedule changes to schedulers or supervisors. Operations: Attend in‑service training and mandatory agency meetings. Requirements Registered Nurse with current state license. Minimum one‑year experience as an RN in an acute care setting.
