TLC Nursing is seeking a travel nurse RN Skilled Nursing for a travel nursing job in Wray, Colorado.

Provide day-to-day nursing care to residents of a long-term care facility, manage chronic conditions, and support rehabilitation and palliative goals as appropriate. Focus on safety, comfort, and quality of life in a geriatric population.

Collaborate with physicians, therapists, and family members to develop and adjust care plans, discharge planning, and transitions to higher levels of care when needed.

Responsibilities

• Perform comprehensive resident assessments and monitor chronic diseases, medications, and functional status.
• Administer medications and manage adverse effects, wound care, and basic rehabilitative interventions.
• Coordinate with interdisciplinary team to create and update care plans and goals of care.
• Provide patient and family education on disease management, nutrition, fall prevention, and safety.
• Document care, vitals, medications, and responses in the electronic health record; ensure accurate handoffs.
• Support mobility, activities of daily living, and nutrition support; monitor fluid balance and nutrition intake.
• Participate in quality improvement initiatives and safety events; promote infection control and comfort measures.

  Qualifications Education: ADN or BSN accepted; licensed RN in the United States.

  Licensure: Active RN license in the state of practice; facility-specific certifications as required.

  Certifications:

  Gerontologic Nursing Certification (optional)

  Experience or Skills: in long-term care or SNF settings; strong interpersonal skills and comfort with family communication; proficiency with EMR and care coordination.

  Experience in SNF or geriatric care settings

  Experience with medication administration and wound care

• Experience coordinating with physicians, therapists, and case managers Preferred BSN preferred

  Geriatric certification or continuing education in geriatric care Work Environment Long-term care facility with ongoing resident care; team-based approach; 8- to 12-hour shifts; documentation and care planning occur in the electronic health record; variability in patient acuity. Resident-centered, family-inclusive environment with emphasis on safety and comfort; routine observations, progress notes, and care plan updates.
