About the Position

Job Title

Physical Therapist – Home Health

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across the East Portland area and surrounding communities
• Conduct patient evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the home environment
• Complete clinical documentation using EPIC Remote Client and Rover mobile application, including OASIS assessments
• Manage a caseload of up to 6 patient visits per day, meeting standard productivity expectations
• Coordinate care and communicate effectively with the broader care team to support patient outcomes

Required Qualifications

• Active or pending Physical Therapist license in the state of Oregon
• Minimum of 1 year of skilled home health experience
• Current Basic Cardiac Life Support (BCLS) certification
• Valid driver's license and reliable personal transportation required
• Ability to travel within a service area with an occasional maximum radius of approximately 60 miles per day

Preferred Experience / Skills

• Familiarity with EPIC electronic health record systems in a home health context
• Experience with OASIS documentation and home health productivity standards
• Strong time management skills with the ability to efficiently navigate a multi-patient daily schedule
