Job Description
Registered Nurse - Hospice

Schedule

Day shifts, Monday to Friday, 8:00 AM - 5:00 PM, with flexibility in start and end times. On-call duty is required for one night shift every other week.

Assignment Details

This is a home hospice position with an average daily census of 60 patients. The contract lasts for 13 weeks with a guarantee of 3 shifts per week.

Requirements

• 1 year of experience required
• BLS certification
• Current nursing license in the relevant state(s)
• Experience in Home Health/Hospice
• Reliable transportation is necessary

Responsibilities

• Manage a caseload of 14-16 patients or assist other case managers with routine visits.
• Perform patient visits 1-2 times per week, typically lasting around 1 hour each.
• Provide care to patients with various diagnoses, including end-of-life care for patients with 6 months or less prognosis.
• Document patient care and maintain accurate records using electronic medical records (EMR) system.

Compliance / Notes

There are no restrictions on time off (RTO) beyond a maximum of 7 days during the assignment. Local candidates must reside a minimum of 75 miles from the facility.
