Registered Nurse (RN) - Home Health Infusion - $72K-117K per year

Job Details Registered Nurse (RN) – Home Health Infusion.Location: Valley Park, Missouri.Employment Type: Staff, full‑time (40 hours per week). Day shift primarily 8:00a.m. – 4:30p.m., with flexibility for operational and patient needs. Weekend, holiday, and on‑call rotations as required.Regular local travel throughout the BJC Home Care service area is required. Mileage reimbursement is provided in accordance with IRS guidelines. Responsibilities Assess patient preferences and barriers to care, considering values, emotional, spiritual, cultural, and population‑specific needs. Develop, implement, and document individualized care plans with defined goals in collaboration with the interprofessional team and patient/family/caregiver. Communicate care plans across the continuum of care. Promote respect, equity, and empathy in interactions with diverse and vulnerable populations. Practice collaborative problem‑solving, service recovery, and advocacy for patient‑family centered continuity of care. Integrate data from the interprofessional team and apply critical thinking to ensure safe and timely care. Evaluate changes in patient condition; inform and collaborate with family and the interprofessional team; update the plan of care in the EMR. Ensure evidence‑based practice and quality patient outcomes. Qualifications Education: Nursing Diploma or Associate’s degree in Nursing (minimum). Experience: Less than 2 years in nursing; no supervisory experience required. Licenses & Certifications: Current RN license and valid driver’s license. Preferred: Bachelor’s degree in Nursing or Home Health and 2–5 years experience. Additional: Must register with the Missouri Family Care Safety Registry.
