Service Coordinator IJob Purpose:Coordinates and monitors services for individuals with developmental disabilities by developing person-centered plans, advocating for needed supports, maintaining accurate documentation, and ensuring compliance with Targeted Case Management requirements.Key Responsibilities:Manage a caseload by coordinating services for individuals with developmental disabilities.Develop person-centered plans tailored to the unique needs of each individual.Monitor outcomes to ensure that services provided are effective and meet the individual's goals.Maintain accurate and up-to-date documentation of all services and interactions.Meet productivity standards set by the organization.Advocate for individuals to ensure they receive necessary supports and services.Ensure compliance with Targeted Case Management (TCM) requirements and standards.Required Education:A Registered Nurse license; or a Bachelor’s degreeRequired Skills and Abilities:Strong organizational skillsEffective communication skillsTime management abilitiesCritical thinking skillsProblem-solving capabilitiesAttention to detailAbility to work independently and as part of a teamProfessionalism in all interactionsPreferred Skills and Abilities:Proficiency in documentation practicesAdaptability to changing situationsComputer proficiencyExcellent customer service skillsRelationship building capabilitiesAdvocacy skillsKnowledge of person-centered practices #J-18808-Ljbffr Paycom - ATS
