Office Location: 325 N Corporate Dr Suite 260, Brookfield, WI 53045 Coverage area: Travel throughout the territory is required- Primarily Waukesha and Milwaukee Counties

Schedule: Full Time Monday-Friday 8AM-5PM

As a RN Case Manager You will:

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
