Case Manager

We’re looking for a compassionate, resourceful Case Manager to support a senior housing community in Buellton, CA. This is a hands-on, on-site role where your work directly impacts people’s ability to remain safely and successfully housed. What You’ll Be Doing Deliver individualized, strengths‑based case management grounded in each resident’s unique needs Connect residents to critical community resources (mental health care, financial assistance, senior services, veteran supports, and more) Partner closely with outside caseworkers, agencies, and community organizations to build strong support networks Support residents as they successfully transition into housing after homelessness Help individuals navigate challenges related to health, housing stability, and organization/hoarding concerns with compassion and respect Foster connection and belonging by supporting on‑site community activities like meetings, groups, gardens, classes, and wellness events What We’re Looking For Bachelor’s degree (required) or Master’s degree (MSW, MFT, or related, preferred) A minimum of 3 years of case management or related social services experience (depending on education level) Experience working with seniors, formerly unhoused individuals, or high-acuity populations is strongly preferred Comfortable working independently while collaborating with external partners Strong communication, organization, and follow-through skills Spanish speaking is a plus, but not required Benefits include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, an EAP program, commuter benefits, and a 401K plan.
