Job Overview Registered Nurse – Home Health position at Elara Caring provides high‑quality patient care in the patients’ homes. Responsibilities Ensure all activities align with the vision of Elara Caring’s leadership. Maintain patient and staff privacy and confidentiality pursuant to HIPAA. Assess assigned case load, identify patients’ physical, psychosocial, and environmental needs. Assume primary responsibility for caseload: assess, plan, coordinate, implement, and evaluate the plan. Integrate data on physical, psychological, social, and spiritual factors into the nursing care plan. Conduct initial and follow‑up evaluations, adjusting visits per patient, family, and facility needs. Communicate significant findings or changes to the Clinical Supervisor, physicians, and other team members. Revise the plan of care in response to identified issues and notify the team. Required Qualifications Current, unrestricted state RN license. 1+ year experience in a clinical care setting. Willing and able to travel within the coverage area; dependable vehicle, valid driver’s license, and auto insurance. Associate’s or Bachelor’s degree in Nursing preferred. Previous Home Health or Hospice experience preferred. Experience with HomeCare HomeBase preferred.
