Job Code: HCP071/Registered Nurse 2 or T7001 Professional Health Care Worker Shift: Full Time Job Hours: Times May Vary Base Location: Clayton (travel to worksites across Gwinnett, Rockdale, Newton Counties) Division/Department/Program: Intellectual Developmental Disability (IDD) Services Full-time X Part-time PRN Exempt Non-Exempt X The Registered Nurse (RN) for IDD Services provides oversight of health services across multiple counties, including Gwinnett, Rockdale, Newton, and Clayton. This role is responsible for delivering high-quality, person-centered nursing care to individuals with intellectual and developmental disabilities (to include co-occurring conditions), ensuring services are respectful of each individual's preferences, strengths, and goals. Working within a trauma-informed and integrated care framework, the RN collaborates closely with interdisciplinary teams, direct support staff, families, and community providers to coordinate comprehensive medical and behavioral health services. The RN promotes a holistic approach to care that addresses physical health, behavioral health, and health-related social needs of well-being, while ensuring adherence with Georgia DBHDD standards and Medicaid waiver requirements. This position supports continuity of care across multiple service sites by providing nursing assessments, care planning, staff training, and oversight of medication and health-related services. The RN plays a critical role in fostering safe, supportive environments, improving health outcomes, and enhancing quality of life for individuals served throughout service locations. Duties & Responsibilities: • Provides professional nursing care to individuals with intellectual and developmental disabilities (IDD) in accordance with Georgia DBHDD, Medicaid waiver (NOW/COMP), and agency standards. • Conducts comprehensive nursing assessments, including initial and ongoing evaluations of physical health, medical history, medications, functional status; and completes required Health Risk Screenings and nursing reviews. • Develops, implements, and updates individualized nursing care plans in coordination with the Individual Service Plan (ISP), physician orders, and interdisciplinary team recommendations, ensuring person-centered and clinically appropriate care. • Delivers and oversees direct nursing care, including medication administration and management, monitoring for side effects, administering treatments (e.g., injections), and providing instruction on self-care and health maintenance. • Monitors client health status through routine observations, vital signs, and review of lab results; promptly identifies, documents, and reports changes in condition, adverse reactions, or new symptoms to appropriate providers. • Ensures safe medication practices in compliance with the Georgia Nurse Practice Act and DBHDD standards, including proper documentation, storage, and supervision/delegation of medication administration to trained staff. • Maintains accurate, timely, and compliant clinical documentation, including assessments, progress notes, incident reports, physician orders, and health records, meeting VPH policy and external review and accreditation (e.g. CARF) requirements. • Provides education, training, and ongoing support to staff, individuals served, and families regarding health conditions, medications, infection control, and other healthcare needs. • Coordinates medical care with physicians, specialists, pharmacies, and other providers to ensure continuity of care and appropriate follow-up services. • Participates actively in interdisciplinary team meetings, offering clinical input, recommending care plan modifications, and supporting overall treatment outcomes. • Responds to medical emergencies and crises, implementing appropriate interventions and ensuring timely communication and follow-up. • Completes and follows up on incident reporting (e.g., medication errors, injuries, significant health events) in accordance with DBHDD policies and quality assurance standards. • Supports compliance and quality improvement activities, including audits, surveys, and adherence to DBHDD and organizational policies. • Works effectively in a fast-paced environment and may provide on-call or after-hours nursing support as assigned. • Performs other duties as assigned. Minimum Qualifications: • Bachelor's degree in Nursing OR Associate degree in Nursing from an accredited program • Current, active Georgia Registered Nurse (RN) license • Current First Aid/BLS certification Preferred Qualifications: • One (1) or more years of nursing experience • Experience with IDD populations or developmental disability services (NOW/COMP waivers) • Experience in behavioral health or community-based care settings • Knowledge of DBHDD standards, ISP process, and medication administration requirements Requirements/Competencies: • Must be able to lift 20 pounds. • Requires long periods of sitting/standing. • Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy. • Candidates for selection must pass a criminal background check (including fingerprinting). • Must pass a pre-employment drug screen and subject to random drug screens. • Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license. • Other duties as assigned.
