Requirements

Years of Experience: 2 years, acute Hospital Case Management experience performing Care Coordination and discharge planning
EMR: EPIC 
Travel Exp Req:  Yes
Experience with HMOs, IPAs, etc.
Experience with advance directive, POLST, protective services process 
HARDSTOP: ACUTE CARE Hospital Case Management EXPERIENCE AND MUST BE CURRENT WITHIN THE LAST YEAR.
