About the Position

The Physical Therapist provides specialized physical therapy services within the home health and hospice environment, delivering comprehensive, patient-centered care while ensuring accuracy, safety, and compliance with all regulatory standards. This role provides rehabilitative services to home health and hospice patients, carrying out skills that support treatment, rehabilitation, safety, maintenance, and education for patients and their families, with particular attention to the geriatric population.

Requirements

Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans. - Supervises and coordinates care with PTAs. - Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence. - Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning. - Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs. Qualifications: - Licensed Physical Therapist in North Dakota. - Current driver's license required and car available to travel to patient's home.
