Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum of 2 years of acute medical social work experience performing both social work and discharge planning License: CA state license required; pending license accepted, but must be licensed upon start date Certifications: MSW or LCSW required Must-Have: - Knowledge of care coordination and collaborative care - Ability to develop a plan of care to meet patient and caregiver needs - Experience in discharge planning and medical surgical care - Skills in needs assessment and ordering DME - Participation in care conference activities - Ability to organize and participate in family meetings - Knowledge of drug and alcohol abuse treatment options

Description: Floating is not required. First-time travelers must be screened. The patient ratio during days is 1:25. Weekend requirements typically include one full weekend every four weeks, based on department needs. The facility is an acute care facility. Daily patient census can go up to 25 patients. Knowledge of mental health history, crisis intervention, advance directives, and protective services processes is essential. Participating in the development of treatment plans is a key responsibility. Travelers are expected to float between all facilities within a state. Refusal to float may result in termination of assignment.
