• Key Requirements Summary: 1. Experience: 2 years of acute medical Social Work experience required, performing both roles of Social Work and discharge planning 2. Cases: Short-term acute care inpatient; adults and geriatrics; case load up to 25 patients; knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, and protective services process required 3. Certifications: MSW or LCSW (either/or MSW) required 4. License: CA state license required; pending license accepted but must be licensed by start date 5. Education: Not specified 6.
