About the Position The Clinical Consultant for the Enhanced Care Management (ECM) – Community Supports (CS) program supports programs and advocates by ensuring the quality of clinical service. The consultant maintains accurate, current clinical records that meet Medical Assistance standards and regulations, provides expertise and guidance to improve client care, program effectiveness, and clinical outcomes. A strong clinical background in care management and a passion for innovative practices are essential. What You Will Do Job Summary The ECM Clinician is responsible for member coordination, improving health outcomes, enhancing satisfaction, and reducing unnecessary healthcare utilization. ECM is a comprehensive, patient‑centered approach that coordinates medical, behavioral, and social services for complex high‑need populations. Essential Duties Review, approve, and provide clinical sign‑off on comprehensive assessments, individualized care plans, and treatment‑related documentation. Ensure documentation meets medical necessity and ECM/CalAIM standards. Provide feedback to staff to improve clinical quality, accuracy, and compliance. Maintain consistency in clinical practices across multiple contracts, counties, and Managed Care Plans. Serve as a clinical resource and consultant to ECM staff and multidisciplinary teams. Guide complex and high‑risk cases, including behavioral health and substance use concerns, crisis intervention and stabilization, and treatment planning. Support staff with case debriefing and clinical guidance. Assist with escalated clinical involvement and consult on clinical and administrative issues. Help develop and implement clinical strategies aligned with program goals. Integrate evidence‑based practices into ECM service delivery. Evaluate and recommend clinical models, care pathways, and best practices to enhance outcomes. Collaborate with Program Director and leadership to support program design and clinical effectiveness. Support implementation of Teaching and Therapeutic Community models. Ensure ECM services meet local, state, and federal regulations, Managed Care Plan requirements, and contractual expectations. Conduct chart reviews, audits, and quality checks. Identify gaps, trends, and risks in clinical service delivery and recommend improvements. Support audit readiness, site visits, and compliance reviews. Develop and deliver training on clinical documentation standards, care coordination techniques, and best practices in ECM service delivery. Support onboarding and ongoing staff development and clinical competency building. Promote consistency in clinical approaches across programs and counties. Serve as a liaison between ECM programs, healthcare providers, and community partners. Coordinate care with multidisciplinary teams to improve service delivery. Participate in committees and meetings to advocate for ECM services and the populations served. Support quality improvement initiatives with leadership and care teams. Provide clinical supervision and guidance to ECM Advocates and Housing Navigators (as needed). Offer ongoing consultation, mentorship, and support without direct managerial responsibility. Guide staff in applying trauma‑informed care and client‑centered approaches. Attend meetings, trainings, and workshops as requested. Maintain documentation of clinical reviews and approvals. Provide reports on clinical trends, outcomes, and recommendations. Perform other duties as assigned. What You Will Bring Required Master’s or Doctoral degree in behavioral health, public health, or related human services field. Active, unrestricted licensure in California: LCSW, LMFT, LPCC, PsyD, or PhD. Minimum 5 years of clinical experience in behavioral health and high‑acuity or underserved populations. At least 2 years of experience in care management, population health, or ECM‑related services. Strong experience in clinical documentation review, treatment planning, and crisis intervention. Valid and unexpired driver’s license. Preferred Previous clinical experience with individuals recently released from jail or prison; supervisory experience. Experience providing services inside correctional facilities. Knowledge of re‑entry systems, benefits navigation, and housing resources. Experience with ECM/CalAIM programs and Managed Care Plans. Experience working with justice‑involved or re‑entry populations. Previous supervisory or clinical oversight experience. Knowledge of housing resources, benefits navigation, and community‑based systems. Comfortable working in both secure custodial environments and community‑based field settings. Vehicle, car insurance, and valid driver’s license. Bilingual Spanish/English fluency (oral and written) highly desirable. Certifications or Licenses Licensed with the California Board of Behavioral Sciences (BBS) (e.g., ASW, AMFT, LCSW, LMFT). Community Health Worker certification through California’s HCAI Department (preferred). Driver’s License. Skills and Abilities Excellent interpersonal, communication, and facilitation skills with diverse stakeholders. Expert knowledge of clinical practices, care coordination, and patient‑centered care. Strong analytical skills to interpret clinical and utilization data and translate findings into actionable strategies. Advanced understanding of medical necessity criteria, ECM/CalAIM requirements, behavioral health systems, and regulatory frameworks. Strong clinical judgment, assessment, and problem‑solving abilities. Knowledge of trauma‑informed care, re‑entry services, and justice‑involved populations. Ability to work independently while supporting multiple programs and interdisciplinary teams. Collaboration skills across counties, providers, and multidisciplinary teams. Cultural competency and ability to work with diverse and underserved populations. Knowledge of contract parameters, deliverables, and compliance with regulations. Ability to support quality assurance efforts, identify gaps, and recommend improvements. Ability to manage multiple priorities, adapt, and implement effective solutions. Clear communication of concepts, preparation of reports, and development of policies and procedures. Proficiency in Electronic Health Records (EHR) systems and Microsoft Office Suite.
