divh2NURSE PRACTITIONER IV-HOSPITALIST-MT PLEASANT/h2pJob Category: NURSING Requisition Number: NURSE025305/ppFull-Time Mt Pleasant Boro Mount Pleasant, PA, USA/ph3Description/h3pPerforms history and physicals as designated by the physician./ppIdentifies normal and abnormal findings and reports to the physician in a timely manner./ppProvides preoperative teaching to patients and or families./ppPerforms patient rounds ensuring accurate assessment, review of labs and diagnostic procedures, and reports overall progress to supervising physician./ppOrders appropriate testing per established guidelines and as outlined by supervising physician./ppDocumentation supports a review of lab/diagnostic results, is reflective of all required history and physical assessment elements./ppCommunicates with supervising physician in a timely manner regarding changes in the patients progress, status, or critical test results./ppPrepares for and develops discharge planning in conjunction with supervising physician./ppDictates discharge summaries timely and appropriate./ppProvides counseling and instruction to patients and or families as appropriate./ppPerforms clinical responsibilities in a manner that maximizes patient care and outcomes./ppAssumes appropriate code team role during emergency situations and responds effectively./ppFacilitates timely treatment of abnormal labs or other relevant findings./ppDevelops workflow patterns to facilitate patient transfers or discharges./ppParticipates in outcome management and Press Ganey scores./ppEvaluates patients current status providing recommendations for ongoing treatment./ppProvides patient/family education regarding medication, rationale for use and possible side effects/ppCollaborates with the supervising physician and/or substitute supervising physician and members of the treatment team to achieve desired outcomes and to assure continuous flow of patient care./ppAlerts the treatment team to any significant changes in the patients status./ppWithin prescribed time frames, review treatment goals and objectives with patients./ppCommunicates pertinent information efficiently to the supervising or substitute supervising physician./ppDocuments all screenings and assessments performed, including patients mental status./ppRecords medication changes, prescriptions written and samples provided./ppDocuments patients complaints, concerns and questions as appropriate./ppDemonstrates leadership by utilizing the concepts inherent to the practice of a professional APP./ppParticipates in Performance Improvement activities designed to enhance the quality of patient care, customer service, or practice efficiencies./ppIdentifies initiatives to improve patient outcomes and satisfaction./ppAssists in chart review/audits as needed for performance improvement data collection./ppSupports initiatives implemented to improve patient outcomes and or efficiencies./ppProvides for patient safety in compliance with hospital and or physician practice policies./ppAdheres to infection control processes, including standard precautions, to reduce the risk of acquiring or transmitting infections./ppANCC Acute Care Certification required/ppCRNP experience preferred/ppEqual Opportunity Employer/Protected Veterans/Individuals with Disabilities This employer is required to notify all applicants of their rights pursuant to federal employment laws.
