Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities. The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs. The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet. St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles. St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Job Summary

The Case Manager I - Workforce Development provides integrated, healing-centered case management services to participants across all three cohort tracks of the Digital Technology Workforce Development Program: a women’s healing-centered cohort, a co-ed cohort including justice-experienced adults and individuals with histories of homelessness, and a transitional age youth cohort (ages 18–30).

Essential Duties and Responsibilities

Key Areas of Responsibility:

• Conduct comprehensive, trauma-informed intake assessments and develop individualized, goal-oriented service plans for each participant across all three cohort tracks, identifying both barriers and strengths.
• Provide ongoing, individualized case management through regular one-on-one meetings, check-ins, and responsive outreach, maintaining meaningful contact with every participant across all three cohorts.
• Provide advocacy and linkage to needed or supportive services and resources including mental health services, rental assistance, food access, benefits enrollment, re-entry legal aid, childcare, immigration legal services, and transportation.
• Respond to urgent requests for assistance from participants and manage crisis response across multiple population types with appropriate clinical judgment, safety planning, and coordination with licensed mental health and crisis staff.

Essential Duties:

• Participate in and help facilitate regular multi-disciplinary case conferences with the Program Manager, Career Coach, and an expanded network of specialists including: youth development counselors, licensed mental health clinicians, gang intervention and street outreach counselors, re-entry case managers, probation liaisons, TAY housing specialists, and other SJC program staff.
• Work closely with re-entry case managers, probation officers, and legal service providers to ensure justice involved participants receive coordinated support that addresses both reintegration needs and workforce development goals.
• Maintain accurate, timely, and comprehensive case notes and participant records across all three cohort tracks in SJC’s data management systems.
• Ensure compliance with program, agency, and funder requirements related to client data, confidentiality, and documentation standards.

Qualifications

Knowledge, Skills & Abilities:

• Strong knowledge of and experience applying: Trauma Informed Care, Healing-Centered Engagement, Harm Reduction, Motivational Interviewing, Strengths-Based Case Management, and Crisis Intervention.
• Ability to work effectively in multi-disciplinary team environments, engaging clinical staff, youth counselors, gang intervention specialists, and instructional staff as collaborative partners.
• Skilled in conflict resolution, de-escalation, and safety planning across diverse and sometimes high-risk participant circumstances.
• Excellent communication, interpersonal, and documentation skills; able to balance a high volume of participant contacts while maintaining professional case records.

Experience:

• 3+ years of experience with highly vulnerable populations in a non-profit, social services, or workforce development setting.
• Demonstrated experience serving at least two of the following populations: justice-involved/re-entry individuals, survivors of domestic violence, people experiencing homelessness, and/or transitional age youth (TAY).
• Experience participating in or facilitating multi-disciplinary case conferences.
• Experience designing and facilitating group workshops or life skills sessions.

Education:

• Bachelor’s degree from an accredited college or university in social work, human services, or a related field. Master's degree strongly preferred. 

Direct Reports:

This position has no direct supervisory responsibilities. 

Computer Skills:

• Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
• Google Suite — Gmail, Drive, Docs, Sheets, Slides, Meet
• Ability to use widely supported internet browsers.

Certificates, Licenses and Registrations:

• None

Travel Requirements:

• Access to reliable transportation.
• Travel to meet with clients as needed.
• Travel between St. Joseph Center sites and at community partner locations as needed.
