Job Title: Physical TherapistDepartment:Job Summary: The Physical Therapist provides evaluation, treatment, and management of patients with physical impairments, functional limitations, musculoskeletal injuries, neurological conditions, developmental delays, and chronic pain. The Physical Therapist works collaboratively with providers and care teams to improve patient mobility, function, independence, and overall quality of life through evidence-based therapeutic interventions. This position supports the mission of Chiricahua Community Health Centers, Inc. by delivering high-quality, patient-centered care in a community health center setting. Based on organizational needs, services may be provided to pediatric, adolescent, adult, and geriatric populations. Qualifications and Requirements:The requirements listed below are representative of the knowledge, skill, and/or ability required. Job duties may be modified at any time based on business needs. Essential Job Duties:Direct Patient CarePerform comprehensive physical therapy evaluations and develop individualized plans of care.Assess patient functional status, strength, range of motion, mobility, balance, coordination, pain, and rehabilitation needs.Establish measurable treatment goals and modify treatment plans based on patient progress.Provide skilled physical therapy interventions utilizing evidence-based practices.Evaluate and treat patients with orthopedic, neurological, developmental, post-surgical, and chronic health conditions.Reassess patient progress regularly and determine the need for continued therapy services.Design and implement home exercise programs to support patient recovery and long-term success.Recommend adaptive equipment, assistive devices, and community resources as appropriate.Educate patients, families, and caregivers regarding diagnoses, treatment plans, injury prevention, and self-management strategies.Coordinate care with primary care providers, specialists, behavioral health professionals, schools, community agencies, home health providers, and other healthcare professionals as needed. Documentation & ComplianceComplete timely and accurate documentation in the Electronic Health Record (EHR) in accordance with organizational, state, federal, payer, and professional standards.Maintain treatment records, evaluations, progress notes, discharge summaries, and other required documentation.Ensure compliance with HIPAA, HRSA, Medicare, Medicaid, commercial payer, and organizational requirements.Participate in documentation audits and quality assurance activities as requested.Support prior authorization, referral, and care coordination processes when applicable. Team & Organizational SupportParticipate in interdisciplinary care team meetings and case consultations.Contribute to quality improvement and patient safety initiatives.Maintain licensure and continuing education requirements.Participate in staff meetings, trainings, and mandatory education programs.Promote rehabilitation services through patient, provider, and community education.Perform other duties as assigned to support clinical operations and organizational needs. Required Education, Experience, Certificates & Licenses:Graduate of an accredited Physical Therapy program. Current Arizona Physical Therapist license in good standing. Current CPR certification. Valid Fingerprint Clearance Card. Valid driver's license and proof of insurance if travel between sites is required. Ability to meet continuing education requirements necessary to maintain licensure. Preferred Education, Experience, Certificates & Licenses:Doctor of Physical Therapy (DPT) preferred.Two (2) or more years of clinical experience. Experience in community health centers, outpatient rehabilitation, pediatrics, orthopedics, or neurological rehabilitation. Bilingual in English and Spanish. Required Language Skills:Ability to comprehend and compose instructions, correspondence, and communications in English in both oral and written format. Adept at reading, writing, and interpreting technical documents and procedure manuals. Ability to effectively present information in one-on-one and small group situations to patients, internal providers and staff and other agency staff working in cooperation with the organization. Physical Requirements:Healthcare clinical work environment. Frequent standing, walking, bending, reaching, lifting, transferring, and assisting patients during treatment activities. Ability to exert up to 50 pounds occasionally and assist with patient mobility and transfers. Frequent use of therapy equipment, computer systems, and office technology. Possible exposure to communicable diseases, bodily fluids, and hazardous materials. Travel between clinic locations may be required.Possesses close visual acuity necessary to accurately record and view information on a computer monitor, handwritten and typed documents. Ability to discern the nature of sounds at a normal spoken volume. Other Required Knowledge, Skills, and Abilities:Knowledge of physical therapy principles, evaluation techniques, treatment modalities, and evidence-based rehabilitation practices. Strong clinical reasoning and problem-solving abilities. Ability to develop individualized treatment plans and measurable outcomes. Excellent interpersonal, communication, and patient education skills. Ability to work independently while collaborating effectively within a multidisciplinary team. Knowledge of medical terminology, anatomy, physiology, and rehabilitation practices. Proficiency with Electronic Health Records and Microsoft Office applications. Ability to maintain patient confidentiality and adhere to professional ethical standards. Ability to perform a variety of assignments and make decisions requiring considerable independent judgment. Displays sound and accurate judgment and ability to make timely decisions. Knowledge of HIPAA and Corporate Compliance rules and regulations. Strong Computer proficiency required with proficiency in use of all Microsoft Office programs. Excellent interpersonal skills with a commitment to quality healthcare and customer service. Work Environment & Conditions:Work environment is typical of an clinical office setting with no substantial exposure to adverse environmental conditions. Work requires reliable transportation as position may require travel.Work requires the ability to be contacted by cell phone during or outside of regular work hours.
