Outpatient Occupational Therapist, (OT) – Home Health Cary, NC 27511 | 35.766900883 | -78.782078782 Responsibilities Perform outpatient therapy evaluations to develop a plan of care for patients in senior living communities or group homes for adults with intellectual disabilities. Operate at the top of licensure utilizing research‑based approaches to maximize functional independence. Collaborate with home‑health therapy partners to transition care from home health to outpatient services, achieving client‑centered goals. Educate clients, families, and community caregivers on care plans to promote functional independence. Perform evaluations, treatments, and discharge planning utilizing the outpatient EMR platform. Qualifications A current North Carolina Occupational Therapist license. Prior home‑care experience preferred, but not required. Minimum of one year of recent work experience as an Occupational Therapist. Graduated from a program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills. Solid computer skills; prior experience with electronic medical records preferred. Benefits Enjoy being part of a caring team.
