RN Case ManagerCommunity Care is hiring a RN Case Manager to join our Family Care Program team. This is a full-time opportunity working out of our Dane County location. $10,000 Sign On BonusRN Case Manager Job Responsibilities:Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.RN Case Manager Job Requirements:Education: Associates degree in Nursing Required, Bachelors Degree preferred.Licensure: Wisconsin Licensed Registered NurseLicensure: Eligibility for long-term care functional screener certification is requiredLicensure: WI Drivers LicenseExperience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required Preferred 2 years of work in community nursingRN Case Manager Job Schedule:Monday- Friday 8:00 AM-4:30 PM.About Community CareCommunity Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community.
