Salary: $52,000–$57,000 annually, based on experience and qualifications

As a Resident Case Manager (RCM), you will lead a team of Direct Support Professionals while partnering with individuals, families, guardians, healthcare providers, and community partners to ensure the people we support receive high-quality, person-centered services.

What You'll Do

Lead and Develop Your Team

Our Direct Support Professionals are the heart of our organization. As an RCM, you'll create an environment where employees feel supported, appreciated, and equipped to do their best work.

Responsibilities include: 

• Training around personal cares and care plans, track employee training effectiveness and corresponding documentation
• Leading Team Meetings, coaching and mentoring staff
• Providing regular feedback, recognition, and professional development
• Creating a culture built on respect, accountability, and teamwork
• Supporting employees through challenges while maintaining clear expectations
• Participating in performance management and employee development
• Helping ensure homes are appropriately staffed and supported

Coordinate Person-Centered Supports

You'll serve as the primary coordinator for the individuals living in your assigned homes by:

• Coordination medical appointments, behavioral supports, and other services as well as support individuals through every phase of life including end of life care
  
• Completing assessments, service plans, documentation, and required reporting
  
• Monitoring health, safety, finances, and overall quality of life
  
• Ensuring services meet agency standards and funding requirements

Support Individuals to Live Meaningful Lives

You'll help ensure individuals:

• Receive individualized supports that reflect their goals and preferences

Direct support may occasionally be provided during staffing shortages or emergency situations.

Administrative Leadership

You'll also:

• Maintain accurate documentation and confidential records
• Oversee household operations and participant funds 
• Participate in quality improvement initiatives
• Serve as part of our shared after-hours on-call rotation
• Perform other related duties as assigned

Qualifications

Required

• Three years of experience supporting individuals with disabilities, older adults, or other vulnerable populations, or an equivalent combination of education and relevant professional experience
• Previous experience leading, mentoring or coordinating the work of others
• Strong communication, organization, and problem-solving skills
• Ability to build positive working relationships with employees, families, guardians, healthcare professionals, and community partners
• Valid Wisconsin driver's license, reliable transportation, and automobile insurance
• Ability to successfully complete required background checks

Preferred

• Bachelor's degree in human services, Social Work, Psychology, Healthcare Administration, or related field
• Experience in residential services, case management, or long-term care 
• Knowledge of Wisconsin Medicaid - funded programs, including Family Care and IRIS

Physical Requirements

This position requires the ability to lift, bend, reach, kneel, assist individuals with activities of daily living when necessary, and occasionally provide direct support during emergencies or staffing shortages.

Schedule 

This is a full-time leadership position.

Resident Case Managers participate in a shared after-hours on-call rotation to ensure consistent support for our homes. We are committed to continually improving our on-call systems to promote a healthy and sustainable work-life balance for our leadership team.
