Physical Therapist – Home Health

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across the West Portland area and surrounding communities
• Conduct patient evaluations and develop individualized treatment plans in accordance with physician orders and care goals
• Visit up to 6 patients per day, meeting standard productivity point expectations
• Complete clinical documentation using EPIC Remote Client and Rover mobile application, including OASIS assessments
• Coordinate with the care team to ensure continuity and quality of patient care
• Manage daily scheduling and travel efficiently within an assigned geographic service area

Required Qualifications

• Active Oregon Physical Therapist license
• Minimum 1 year of skilled home health experience
• Current Basic Cardiac Life Support (BCLS) certification
• OASIS documentation experience required
• Primary series COVID-19 vaccination required
• Reliable personal transportation required; mileage reimbursed at IRS standard rate

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for home health charting
• Experience meeting productivity point standards in a home health environment
• Comfort navigating a broad geographic service area with occasional drives up to 60 miles per day
