Case Manager - Rapid ReHousing (Full-time)

About Covenant House California Covenant House California (CHC) is a non‑profit youth shelter that provides sanctuary and support for youth experiencing homelessness, ages 18‑24. We believe that no young person deserves to be homeless; that every young person in California deserves shelter, food, clothing, education, and most importantly, to be loved. CHC offers a full continuum of services to meet the physical, emotional, educational, vocational, and spiritual well‑being of young people, giving them the best chance for success in independence. POSITION SUMMARY CHC is looking for passionate, highly skilled, and energetic Case Managers who possess excellent communication and organizational skills to provide strength‑based, trauma‑informed, and harm‑reduction–focused support to young people. Case Managers will model our mission of respect and unconditional love, and collaborate within a multidisciplinary team to create a safe, inclusive environment for all. ESSENTIAL FUNCTIONS 1. Provide Case Management services with the goal of developing positive relationships with young people through the utilization of the case management system: Intake: Welcome young people, verify eligibility, complete documentation, and conduct expanded assessments with data entry into ETO and HMIS. Individual Success Plan: Develop an Individual Success Plan (ISP) upon completion of assessment and revise it at least every two weeks. Rapid Rehousing Case Management: Assist with locating housing, completing applications, meeting habitability standards, reviewing lease requirements, and coordinating with landlords. Meet weekly and bi‑monthly in‑person, provide mediation, and give written information on tenant rights. Obtain and interpret rental and credit history, develop a household budget, and negotiate debt where possible. Provide referrals and application support for employment, education, insurance, public benefits, and other services. Partner with community and faith‑based organizations to furnish new apartments. Maintain a calendar of work schedules and appointments; monitor progress; coach young people to implement tasks. Maintain knowledge of community resources and make appropriate referrals for services within CHC and to external agencies. Engage in positive transition planning. Document weekly progress notes in the Success Plan in ETO and HMIS, maintain case files, and monitor program outputs and grant documentation. Prepare and present cases at weekly case review meetings and facilitate quarterly Client Success Planning Meetings. Facilitate psycho‑educational and support group activities; provide custodial supervision, ensuring punctuality, cleanliness, and safety of apartments. Provide crisis intervention as necessary. Other duties as assigned by the Santa Clara Program Director. JOB REQUIREMENTS Education Bachelor’s degree in Psychology, Sociology, Social Work, or related field; OR Associate’s degree in the same fields AND 2 years of case management experience, preferably with young people experiencing homelessness. Experience In lieu of a degree, a minimum of 4 years of case management experience, preferably with young people experiencing homelessness. Skills Self‑starter, team player, leadership and program development experience preferred. Knowledge of adolescent development and community resources. Strong organizational communication skills. Bilingual Spanish is preferred but not required. Transportation Valid CA driver’s license, reliable transportation, and good driving record preferred. Ability to navigate public transportation is required; driving CHC vehicles preferable. Working Conditions Meal periods and breaks are required each shift. Weekends and holidays may be required. General office environment. Must lift up to 40 lbs. on occasion. Equipment Used Basic word processing, email, and internet programs.
