Case Manager - ILP

Job Summary:

The Independent Living Program (ILP) provides support to foster youth between the ages of 16 - 21 years old. The Case Manager reports to the Program Manager and is responsible for providing services and engaging youth in the program by performing the following duties:

Essential Duties/Responsibilities:
  • Provide individual, face to face case management to ILP youth focusing on five fundamental components: Permanence and Connection for Independence, Education, Employment, Youth Development and Housing.
    • Assist youth to reach their academic goals
    • Provide support to youth in daily living skills, budgeting, locating and maintaining housing and career planning
    • Collaborate with youth to incorporate their Transitional ILP (TILP) goals into ILP Case Management plan and assist youth in achieving all goals
    • Provide opportunities, connections and resources for affordable, safe housing, within Bill Wilson Center or the larger community.
    • Provide support for parenting ILP clients.
    • Provide referrals to behavioral health, regional center, Department of Rehabilitation or other community resources to meet the needs of the diverse population of youth accessing ILP services.
    • Coordinate or support college/career exploration tours.
  • Manage financial incentives for ILP youth.
  • Provide resources and referrals to relevant, culturally appropriate services in the community where the youth resides or plans to reside.
  • Provide outreach to engage youth not participating in services and youth soon to be eligible.
  • Collaborate with Department of Family and Children Services staff, Juvenile Probation Officers, adult supporters and other stakeholders working with ILP youth.
  • Attend required agency and stakeholder meetings.
  • Ensure that client and program records are maintained and up to date, data entry is expected to be entered the same day as services are provided and no later than 3 business days after services were provided. Provide reports as required.
  • Participate in agency Program Quality Improvement activities and initiatives.
  • Participate in agency Safety initiatives and proactively report safety issues to management immediately.
  • Regular attendance is required.
  • Additional duties as assigned.
Education and Experience:
  • Bachelor's Degree (BA) from four-year College or university in Sociology, Social Work, Psychology, Education, or a related field.
  • 1 years (minimum) case management experience, preferably with TAY youth, foster youth, children, and/or families.
Required Skills/Abilities:
  • Driving is a requirement of the job. Must have possession of current driver's license, access to a vehicle and personal automobile insurance. Current CPR and First Aid Certification required (or complete within 30 days of hire). Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations. TB test required.
  • Must be punctual, efficient and organized; effective and professional under pressure. Ability to balance excellent client care with meeting deadlines and ensuring positive program goals and outcomes. Excellent communication skills - must be personable and informative when presenting Bill Wilson Center programs and services.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Must be able to communicate effectively. Bilingual communication skills required for some Case Manager positions.
  • Ability to work with basic mathematical concepts as required for reporting.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Internet research skills are a plus.

Salary Range:

$25.00 - $28.00 per hour
