Registered Dietitian – Home Nutrition Support

Location: Pharmko – 298 Central Avenue, Lawrence, NY 11559
Employment Type: Part-Time / Full-Time / Per Diem
Compensation: $45 per hour
Reports To: Director of Clinical Services / Lead Pharmacist

Our Nutrition Support Program is dedicated to improving patient outcomes while ensuring safety, comfort, and education throughout the therapy process.

Position Summary:

Pharmko is seeking a dedicated and clinically experienced Registered Dietitian to join our Home Nutrition Support team. The RD will be responsible for assessing and managing the nutritional needs of patients receiving PN and EN, while collaborating with prescribers and pharmacists to optimize individualized care plans. This position also involves patient education, troubleshooting tolerance issues, and representing Pharmko’s nutrition services to referral sources.

Key Responsibilities:

• Conduct thorough nutrition assessments for patients receiving home PN and EN.

• Review labs, weight trends, intake/output, and clinical status to recommend therapy adjustments.

• Collaborate with Pharmko pharmacists and prescribers to implement personalized nutrition regimens.

• Educate patients and caregivers on home nutrition therapy, including formula, pump usage, and monitoring.

• Troubleshoot tolerance and pump issues, and provide timely follow-up.

• Maintain accurate and timely documentation of assessments and interventions.

• Represent Pharmko’s Nutrition Program during outreach efforts to physicians, case managers, and discharge planners.

• Support the business development team with clinical insight and referral support.

Qualifications:

• Registered Dietitian Nutritionist (RDN) credential required.

• CNSC

• Current licensure in New York State (if applicable).

• Minimum of 2 years clinical experience; home infusion or nutrition support preferred.

• Strong knowledge of ASPEN guidelines and evidence-based nutrition therapy.

• Excellent communication, clinical reasoning, and patient interaction skills.

• Ability to work independently and manage a flexible caseload.

Schedule & Flexibility:

• This position offers flexible scheduling , including:

Workload is adjusted based on availability, patient needs, and staffing.

Compensation:

 

• $45 per hour , depending on experience and employment type
