RN Case Manager

Job Summary:

We seek a highly skilled and experienced Registered Nurse with acute care and case management expertise to provide skilled nursing care and coordinate patient treatment plans. The Registered Nurse will perform patient assessments, medication administration, and collaborate with interdisciplinary teams to ensure quality care. This role requires strong communication skills, at least 2 years of relevant experience, and flexibility to float between units as needed. Responsibilities:

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Administer medications and provide skilled nursing care in acute care settings.
• Collaborate and communicate effectively with patients, families, and interdisciplinary healthcare teams.
• Manage case documentation and charting using Epic electronic medical records.
• Float between like units based on patient care needs.
• Participate in weekend shifts every other weekend as required.

Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of 2 years acute care nursing experience.
• Proficiency in clinical assessment, medication administration, and treatment planning.
• Experience with case management and interdisciplinary team collaboration.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.
• Familiarity with Epic electronic health records system.

Preferred Skills & Certifications:

• Bachelor's degree in Nursing or related field.
• Experience in multiple acute care settings such as Medical/Surgical, ICU, ER, or similar.

Special Considerations:

• Required to work every other weekend.
• No on-call responsibilities.
• Must be willing to float between units as needed.
• Free parking on first day; assignment parking provided during orientation.

Scheduling:

• Shift assignments include weekends every other week.
• Shift types and schedules determined by patient care needs and unit requirements.
