Description

Schedule: MondayFriday, 8:00 AM 4:30 PM Weekend Rotation: Every 4th weekend (Friday & Monday off after) Holidays: 2 per year Status: Full-time (40 hours/week)

Job Summary

LPN's provides compassionate hospice and palliative care in home and care-center settings, supporting patients and families with clinical care, symptom management, and education.

Responsibilities

• Provide hospice nursing care (home visits, inpatient support, admissions, end-of-life care)
• Follow Plans of Care and physician orders
• Monitor patient condition and report changes to RN
• Educate patients and families on care and services
• Coordinate care needs, including DME
• Document care accurately in EMR
• Collaborate with interdisciplinary team
• Participate in weekend rotation

Qualifications

Qualifications

• Current Ohio LPN license
• Minimum 2 years LPN experience (Med/Surg, Oncology, or Community Health; hospice preferred)
• Valid driver's license and reliable transportation
• Strong communication and time management skills
• Ability to work independently
