Job Requirements

Position Summary

The Bone Health Nurse Navigator serves as a key member of the multidisciplinary team within the Osteoporosis & Bone Health Clinic, managing the coordination and facilitation of care for patients with fragility fractures or bone quality issues. The Nurse Navigator role is essential for seamless communication between the hospital, outpatient offices, and other providers, ensuring patients receive timely and effective care.

Minimum Requirements

Education

  • BS in nursing
Experience
  • 2 years of experience in a hospital setting
License/Registration/Certifications
  • SC RN License

Preferred Experience
  • 1 year of experience in osteoporosis, fracture care, or bone health
  • 1 year of experience with Fracture Liaison Services, including knowledge of fragility fractures, osteoporosis management, and coordination of care across multiple providers

  • Strong Communication Skills: Ability to effectively communicate with patients, family members, healthcare providers, and insurance companies.
  • Patient Advocacy: A compassionate, patient-centered approach to care, ensuring patients feel informed and supported throughout their treatment journey.
  • Scheduling & Coordination: Strong organizational skills for managing multiple patient schedules, referrals, and follow-ups.
  • Specialty Pharmacy Coordination: Experience in managing medication access, addressing delays, and collaborating with specialty pharmacies to find timely solutions.
  • Technology Proficiency: Proficient in EPIC (EHR system), scheduling software, and database management (e.g., "Own the Bone").
Core Job Responsibilities

Bone Health Clinic Coordinator:
  • Review of Consult List: Assess the list of referrals from orthopedic surgeons for patients with fragility fractures or bone quality issues post-surgery.
  • Chart Review & Patient Assessment: Conduct comprehensive chart checks to evaluate patient history, including medical and family history related to fractures and osteoporosis.
  • Patient Interviews: Interview each patient (either in-person or via phone) to gather detailed information about the fracture and other relevant medical history, including risk factors for osteoporosis.
Clinical Scheduling & Coordination:
  • Bone Density Scan Scheduling: Arrange for bone density scans and coordinate with radiology departments to schedule scans for patients.
  • Osteoporosis & Bone Health Clinic Consultations: Schedule follow-up appointments for patients at the Osteoporosis & Bone Health Clinic, depending on patient acceptance of appointments.
  • Consultation & Follow-Up Documentation: Send consult notes to primary care providers, outlining whether patients accepted or declined appointments, and document these interactions in the clinic's database.
  • Ensure entries are completed at three-month intervals for follow-up care, typically for one year.
Patient Education & Communication:
  • Patient Education: Provide detailed education on the osteoporosis clinic processes, medications, and test results to new and existing patients.
  • Communication of Results: Communicate bone density test results and lab findings to patients, answering any questions regarding their health status.
  • Education on Medications: Inform patients about medication options, including side effects, benefits, and how they play a role in managing osteoporosis.
Ambulatory Nurse Navigation:
  • Cross-Office Liaison: Act as a nurse navigator across all ambulatory Osteoporosis & Bone Health clinic offices.
  • Phone Triage & Call Return: Answer and triage patient phone calls regarding scheduling, concerns, or follow-up questions related to osteoporosis treatment or procedures.
Referral & Scheduling Management:
  • New Patient Referral Management: Review and discuss new patient referrals, explaining the process, what to expect, and scheduling initial consultations and tests.
  • Referral Follow-Up: Monitor and maintain a work queue of new patient referrals, ensuring timely scheduling and follow-up.
Urgent Care & Cancellations:
  • Cancellation Management: Maintain and manage a cancellation list to prioritize and reschedule time-sensitive patients in a timely manner.
  • Urgent Referral Coordination: Facilitate the prompt inclusion of urgent referrals to ensure timely care for patients at risk for further complications.
Liaison with Infusion Centers:
  • Act as a liaison between the ambulatory setting and outpatient infusion centers for insurance sight-of-care issues or other logistical matters.
Medication Management:
  • Follow up support for insurance and billing issues as well as injection training and continuity of care.
Clinical Support for Providers:
  • Provider Assistance: Assist the Osteoporosis & Bone Health providers with various clinical tasks as needed, including chart reviews, patient education, or administrative support.
  • Team Collaboration: Collaborate with providers, clinical staff, and hospital teams to ensure a patient-centered approach and cohesive care plan.
Data Management & Documentation:
  • Database Management: Ensure all patient data, including consult notes, patient history, and follow-up entries, are accurately recorded in the clinic's electronic health records (EHR) and the "Own the Bone" database.
  • Outcome Tracking: Monitor patient progress and compliance with treatment plans, documenting follow-up as appropriate.
