The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes. **Qualifications*** BSN Graduate of NLN Accredited School of Nursing Required or* MS Masters Degree from an accredited school of Social Work Required* 1-3 years Experience in an acute care environment Required* Ability to read, write, and communicate in English.* Strong clinical, organizational, and interpersonal skills.* Registered Nurse New York - New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse.
