The employee is assigned by Enterprise Management Solutions, LLC to provide services to its affiliated organizations in accordance with the duties and responsibilities outlined in this job description.
Enterprise Management Solutions, LLC is responsible for employment administration, including recruitment and hiring, onboarding, payroll processing, maintenance of employment records, human resources administration, and applicable employment-related compliance.
Community Wellness Outpatient Mental Health Center, LLC is the organization receiving the staffing services and is responsible for directing the employee’s day-to-day work activities, operational priorities, worksite procedures, and performance expectations related to the services performed for that organization.
Enterprise Management Solutions, LLC and to its affiliated organizations may coordinate regarding scheduling, performance, workplace matters, regulatory requirements, and other issues necessary to support the employee’s assignment.
The employee remains an employee of Enterprise Management Solutions, LLC while assigned to to its affiliated organizations. The assignment itself does not create a separate W-2 employment relationship between the employee and the assigned organization.

POSITION TITLE: Family Nurse Practitioner 1

ACCOUNTABLE TO : Operations Manager 2
ACCOUNTABLE FOR: Delivering comprehensive, client-centered primary and preventative healthcare services to individuals enrolled in outpatient mental health and supportive housing programs. Responsible for conducting scheduled somatic evaluations, annual physicals, routine follow-up visits, medication management, and patient education while ensuring accurate documentation, appropriate CPT billing, and full compliance with regulatory standards, including COMAR 10.20.06 and 10.21.20.08.
CLASSIFICATION: Non-Exempt (W-2, Hourly) – Part-Time.
WORK SCHEDULE: The employee will work a 24-hour workweek, consisting of three (3) workdays MWF, with work hours between 8:00 AM and 5:00 PM. Work schedules are determined based on business needs, and reasonable adjustments may be made as necessary.
COMPENSATION: $30 - $90 per hour, commensurate with experience and qualifications.
BENEFITS PACKAGE: This position is eligible for standard W-2 employee benefits, including but not limited to those outlined in the Company’s official benefits package. ANTICIPATED TRAVEL : 5% of the time. There is no additional compensation for travel.
SUMMARY OF POSITION RESPONSIBILITIES
To be successful in this role, the employee must be a self-starter, organized, clinically skilled in primary care/somatic healthcare, and collaborative across multidisciplinary teams. As a Family
Nurse Practitioner, you will provide direct primary and preventative healthcare services to clients receiving outpatient mental health and supportive housing services. This includes conducting medical evaluations, prescribing and managing medications, providing health education, coordinating referrals, and monitoring overall physical health in alignment with behavioral health treatment plans.
You are authorized to bill CPT codes 99202, 99203, 99204, 99205, 99211, 99212, 99213, and 99215 for eligible somatic services rendered. In line with COMAR 10.20.06, you are responsible for completing and documenting each client’s somatic status at program enrollment and on an ongoing basis, including medical history, current conditions, prescribed medications, allergies, and recommendations for follow-up. When clients lack a primary care provider, you will ensure referral plans are developed and implemented, with proper consent and collaborative communication between treatment coordinators and providers.
Additionally, under COMAR 10.21.20.08, you will support the integration of substance use disorder treatment with medical care or coordinate with certified providers as needed. You will also ensure that staff are trained and supported in understanding medication safety, infection control practices, and basic medical monitoring procedures.
You will provide leadership and oversight as part of the organization’s medical and clinical governance team, contributing to interdisciplinary care planning, peer review, quality assurance, and regulatory/licensing preparedness.
SCHEDULED DUTIES AND RESPONSIBILITIES
Direct Clinical Care
- Conduct scheduled face-to-face somatic medical evaluations, annual physicals, and follow-up appointments for clients enrolled in Community Wellness programs.
- Perform routine follow-up visits for ongoing medical or medication management (minimum every 90 days).
- Review, document, and update comprehensive medical histories, including chronic conditions, allergies, communicable diseases, medications, and treatment history.
- Prescribe and manage medications within scope of practice following full face-to-face evaluations.
- Provide patient education on wellness, chronic disease management, nutrition, lifestyle interventions, and medication use.
- Educate clients and, when appropriate, caregivers on the purpose, effects, and risks of prescribed medications.
Medication & Monitoring
- Monitor and adjust medication regimens during scheduled evaluations.
- Notify primary care providers of new prescriptions or medication changes at least every six months or as clinically required.
Documentation & Compliance
- Document all clinical encounters in ICANotes within required timeframes.
- Ensure accurate EHR documentation and appropriate CPT coding for evaluation and management services.
- Maintain compliance with COMAR, OHCQ, CARF, Medicaid, and internal standards.
- Accept and review routine lab results and diagnostic reports during scheduled review times.
Team & Organizational Responsibilities
- Participate in scheduled interdisciplinary case reviews, staff meetings, clinical training, and administrative meetings (HR, Scheduling, Patient Access).
- Provide planned support and education to clinical staff regarding medication protocols and service delivery expectations.
- Conduct scheduled peer reviews of prescribers’ work.
- Complete batch documentation audits (treatment plans, biopsychosocials, ASAMs, referrals, intern notes).
- Submit periodic administrative and clinical oversight reports.
- Participate in quality assurance, performance improvement initiatives, and audit preparation activities.
- Maintain ongoing readiness for surveys and regulatory reviews.
UNSCHEDULED DUTIES AND RESPONSIBILITIES
Clinical & Medical Decision-Making
- Order and interpret diagnostic tests and labs as clinically indicated outside of routine visits.
- Adjust care plans promptly in response to abnormal lab results, adverse medication effects, or changes in client condition.
- Address urgent medication concerns, contraindications, or safety issues identified outside scheduled appointments.
Care Coordination & Collaboration
- Facilitate and document coordination with external primary care providers, specialists, hospitals, or agencies as clinically appropriate and with proper consent.
- Collaborate with external treatment providers in cases involving shared clients or complex medical needs.
- Support case managers, therapists, and nursing staff with medical input for emergent or complex integrated treatment planning.
Compliance & Quality Response
- Respond to documentation deficiencies, compliance findings, or corrective action requests identified during audits or reviews.
- Participate in unscheduled quality assurance activities or survey follow-ups as required.
Operational & Clinical Support
- Provide real-time consultation or guidance to clinical staff regarding medication issues or somatic care concerns.
- Review and sign urgent referrals or time-sensitive documentation.
- Address administrative or clinical issues that arise unexpectedly and impact patient care or regulatory compliance.
PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle.
WORKING CONDITIONS : All work is performed in an office where staff is exposed to communicable diseases of patients.
COMPETENCIES AND SKILLS
- Provision of credentialed focused evaluations, treatments and assessments to patients.
- Monitor and evaluate individual treatment plans and service modalities rendered by staff under your oversight with the goal of enhancing effectiveness, streamlining implementation, assuring quality and model fidelity.
- Prescribe and manage medications for patients with focus on somatic/chronic conditions.
- Monitor and evaluate the effectiveness of treatment plans.
- Collaborate with other healthcare professionals to provide comprehensive care.
- Educate patients and their families about somatic health conditions and treatment options.
LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS
Candidates must meet the following minimum qualifications to be considered for this role:

Education
- Master’s Degree in Nursing from an accredited institution
Licensures/Certifications
- Active licensure with the Maryland Board of Nursing (MBON) as a Certified Registered Nurse Practitioner (CRNP)
- Board certification as a Family Nurse Practitioner (FNP)
- Possession of a valid DEA (Drug Enforcement Administration) registration
- Possession of a valid CDS (Controlled Dangerous Substances) registration
- Active CPR/First Aid certification
Experience
- Minimum 2–3 years of clinical experience in primary care, behavioral health, or integrated healthcare settings
