Work Environment

The position is set in a rehabilitation facility located in Keansburg, NJ, where our therapists work collaboratively in a supportive atmosphere focused on achieving the best patient outcomes. Key Responsibilities

• Conduct thorough evaluations of patients recovering from injuries, diseases, or surgeries in a home care environment.
• Develop and implement individualized treatment plans using diverse therapeutic techniques, including exercise prescriptions, stretching maneuvers, manual therapy, and advanced modalities to alleviate pain and enhance mobility.
• Regularly document patient progress, modifying treatment plans as required based on documented responses to therapy and ongoing assessments.
• Conduct follow-up assessments to redefine therapeutic goals and adjust treatment approaches as needed to ensure optimal patient recovery.
• Guide patients and their families on the use of assistive devices and self-care techniques to promote independence in daily activities.
• Educate caregivers and other healthcare team members about relevant physical therapy practices and device applications.
• Maintain current knowledge in the field by attending workshops, engaging in professional networking, and participating in relevant professional societies.

Patient Population / Client Focus

Our patient demographic includes children recovering from sports injuries, adults managing chronic pain, and elderly individuals working on post-operative mobility. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution; a Doctorate of Physical Therapy (DPT) is preferred.
• Current licensure in Physical Therapy from the State of New Jersey.
• Possession of a valid CPR certification.
• Excellent verbal and written communication skills to manage patient interactions and documentation effectively.
• Proficiency in computer skills and word processing for documentation and reporting purposes.
• A flexible and adaptable approach to patient care within a home-based service.

Preferred Experience
