Key Responsibilities of the Patient Care Coordinator:
- Coordinate outreach efforts to patients regarding medication compliance, preventive healthcare needs, and recommended follow up services.
- Support communication between patients, healthcare providers, pharmacies, and other care partners to facilitate timely access to appropriate care.
- Maintain accurate documentation within electronic healthcare systems and tracking platforms while ensuring data integrity and compliance requirements are met.
- Analyze available patient information to identify care opportunities, address barriers to engagement, and assist with appointment scheduling and follow up activities.
- Ensure adherence to privacy, security, and organizational policies while providing professional customer service and administrative support.
- High school diploma, GED, or equivalent required; Associate’s or Bachelor’s degree preferred.
- Minimum of three years of experience within a physician office, hospital, pharmacy, health plan, or similar healthcare environment, along with customer service experience.
- Strong verbal and written communication skills with the ability to effectively engage patients, healthcare professionals, and administrative staff.
- Proficiency with Microsoft Office applications, electronic medical records, web based systems, and general office technology; ability to type at least 45 words per minute.
- Working knowledge of medical and pharmacy terminology, healthcare quality measures, managed care concepts, and healthcare compliance practices.
