Activities Coordinator

Job Overview The Activities Coordinator is a member of the PACE Interdisciplinary team (IDT) and actively participates in all care coordination activities, including assessments and care planning. The role involves designing, implementing, and overseeing a variety of engaging activities tailored to the physical, cognitive, social, and spiritual well‑being of the PACE participants. Responsibilities Participate in all care coordination activities, including assessments and care planning. Design, implement, and oversee activities using techniques such as arts and crafts, animals, sports, games, dance and movement, music, and community outings. Provide a planned schedule of social and other purposeful independent or group activities and promote an engaging environment for participants. Use remote technology as appropriate based on participants’ comfort level, electronic devices, and interests. Adhere to PACE program guidelines, regulatory requirements, policies, procedures, and best practices. Report to and receive direct supervision from the PACE Care Center Supervisor. Minimum Requirements High school diploma or equivalent (minimum). Bachelor’s degree highly desirable. Minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail or elderly population (preferred). Experience with dementia care highly desirable. Completion of 36 hours of Activity Leader (formerly 'Activity Director') course approved by the State of California within 30 days of hire. Bilingual proficiency in English/Spanish/Mandarin/Cantonese, depending on location (preferred). Experience in providing virtual activities preferred. Valid BLS certification or higher, following AHA or American Red Cross guidelines. Compensation Hourly: $25.75 – $32.19.
