RN Case Manager

RN Case Manager-ALPLocated in Brooklyn, NYSalary: $100K to $105K per year (depending on experience)Responsibilities:Will take responsibility for resident care, supervision of staff, working closely with MD and picking up MD orders, writing case notes and ensuring all residents have their 6 and 12 month medical evaluations. Act as the on-site point of contact for operational issues when administration is unavailable.Qualifications:Must have a valid New York RN licenseUAS certification and experience completing UAS assessments strongly preferredMust have prior experience as a Case ManagerMust have experience in a geriatric setting, preferably an assisted living programStrong written and verbal communication skillsOffering an excellent benefit package including health insurance with HRA (NCHHC covers the deductible), dental, vision, company paid life insurance, 403b plan with company match and company annual contribution after 1 year of service.
