Home Assist Health

Home Assist Health is seeking a Community Health Worker in Phoenix, AZ. The CHW will engage clients in 1:1 coaching, develop person-centered care plans, and connect participants with health and social services. The role requires bilingual English/Spanish communication and collaboration with clinical teams to ensure effective program delivery.
