PACE KC Licensed Practical Nurse

As a Licensed Practical Nurse (LPN) for PACE KC, you will be part of a team of caregivers to provide care for the people who need it most. Under the direction and supervision of the Medical Clinic Manager, you provide general and skilled nursing care to participants, execute interventions in accordance with the plan of care, implement primary care orders, evaluate participant complaints in person and/or by telephone, develop appropriate plans of care, provide participant, caregiver and family education and communicate participants’ condition to primary caregivers, physicians and care team. Primary Functions Function as a member of the Care Team. Maintain regular attendance at care team meetings; communicate participant changes, collaborate on plan of care decisions and coordinate for twenty‑four (24) hour care delivery. Effectively communicate with teammates during meetings or one‑on‑one conversations. In coordination with the Medical Care Manager and Registered Nurse and other nursing staff, develop and implement a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintain aligned, comprehensive plans of care for all participants living in residential facilities and coordinate care between facility and Care/Interdisciplinary Team. Perform general and skilled nursing care to participants as assigned. Implement the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observe, record, and report participant’s condition and reaction to drugs and treatments to providers. Triage participants to determine priority of care and assist in the treatment, examination and testing of participants. Participate in nursing on‑call rotation to provide after‑hours and weekend assistance and care, as needed, to PACE KC participants. Receive participant complaints, triage and respond to participant needs. Review and sign off orders and disseminate orders and instructions to appropriate staff and contracted personnel. Maintain and review participant records, charts and other pertinent information. Complete documentation and charting in accordance with organizational policy. Working with the Medical Care Manager, perform care coordination with outside providers and facilities including hospitals, nursing homes, assisted living facilities and physician/provider offices. May accompany participants to specialist appointments or make hospital or other facility visits when necessary for coordination of participant care. Provide treatment instructions and nursing education for participants and family. Maintain accountability for high customer service standards and monitor, evaluate and recommend opportunities for improved participant and family/caregiver satisfaction. Manage and review participant’s records, charts, pertinent information and staff documentation. Document as needed in medical records according to PACE KC standards. Maintain the stability and reputation of PACE KC by ensuring all activities and operations are performed in compliance with local, state and federal laws, regulations and contractual requirements. Establish and maintain a productive working relationship with relevant contracted providers. Responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching and accountability. Follow and ensure that the clinical team follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines. Educate staff regarding care and treatment for the elderly as needed. Act within the scope of his or her authority to practice. Handle potentially infectious specimens with appropriate biohazard precautions and practices of universal precautions. Protect privacy and maintain confidentiality of all company procedures, results and information about employees, participants and families. Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications. Perform other duties as required or requested. Job Related Competencies Ability to read, speak and write fluently in English. Knowledge base of physical, mental and social needs of the frail elderly and their families. Skilled in physical assessment and triaging. Ability to react calmly and effectively in emergency situations. Skilled in preparing and maintaining medical records. Knowledge of nursing standards and best practices. Knowledge of medical equipment and instruments. Competency in PACE KC Emergency Medical Procedures. Strong interpersonal communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations. Familiarity with word processing, data processing and computer entry skills to match existing management and other software used by the program. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in‑servicing and documentation. Ability to apply creative problem‑solving skills to complex issues; ability to plan, prioritize and follow‑up on responsibilities. Ability to foster collaborative working relationships; ability to supervise, direct, coach and develop direct reports and larger team; demonstrated ability to work with a diverse team and flourish in a flexible work environment. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers. Knowledge of infection control practices and quality assurance and improvement. Knowledge of the PACE KC model and philosophy of care. Willingness to provide on‑call coverage within the nursing rotation. Qualifications and Experience Must have a valid driver’s license, proof of insurance and means of transportation. Must have medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact. Education: Graduate of a school of professional nursing. Cardiopulmonary Resuscitation (CPR) and intravenous (IV) certification required. Licensure: Unencumbered license as a Licensed Practical Nurse (LPN) licensed as required by state law. Experience: A minimum of one‑year experience in working with the frail or elderly population. Successful Candidates will have Graduated from an accredited school of practical nursing. Licensed to practice in the State of Missouri and/or Kansas. Basic Cardiovascular Life Support (BCLS), certified. Excellent communication skills, customer service oriented and exhibit the ability to work with people (staff, visitors and clients).
