Implements social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life. Completes required forms and documents in accordance with company policy and state and/or federal regulations. Coordinates discharge planning. Facilitates advanced directive decision-making process. Coordinates Family council. Coordinates grievance complaint process. If appointed Grievance Official will be responsible for overseeing the grievance process. Qualifications For a Social Services Manager Bachelor\'s degree or above in Social Work (BSW) or related field required, such as Human Services, Psychology, Sociology, Gerontology, Special Education, and/or Rehabilitation Counseling. Social Worker license in the state of employment a plus / required (i.e., LBSW, LSW, LCSW, LMSW, etc.). One (1+) years \'supervised social worker experience working directly with elderly individuals in a health care setting required. Prior Long-Term Care, Skilled Nursing, or related healthcare social work experience strongly preferred.
