Hospice Care Liaison (Flexible Hours)

Metropolitan Jewish Health System, Inc. seeks a Clinical Program Liaison to evaluate and plan care for patients referred to Hospice Care services, working on-site at a major medical center. You will join the business development and marketing team and build relationships with physicians, social workers, case managers and discharge planners. Candidates should have a Bachelor’s degree in Social Work and hold LCSW/MSW/LMSW, plus at least two years’ experience in Medical-Surgical or Community #J-18808-Ljbffr Metropolitan Jewish Health System, Inc.
