Job Requirements

• Active Alabama LCSW license required
• At least 1 year of recent acute care or inpatient hospital social work experience required
• Experience in hospital social work, behavioral health, or clinical social work preferred
• Ability to work effectively in a large academic or hospital-based setting
• Comfort floating between units as needed
• Strong communication, collaboration, and patient advocacy skills
• Experience with discharge planning, psychosocial assessments, and community resource coordination preferred

Responsibilities

• Provide inpatient social work services for patients and families
• Complete psychosocial assessments and develop individualized care plans
• Coordinate discharge planning and connect patients with appropriate community resources
• Participate in interdisciplinary rounds and collaborate with the care team
• Support patients through care transitions and address barriers to discharge
• Float to different units as needed to support operational and patient care needs
• Document accurately and efficiently within the Cerner EMR system
