Methods for assessing physical and mental impairments. Understanding the characteristics of illness and expected treatment. Knowledge of benefits, plan contracts, eligibility issues, and requirements. Description: The Care Manager will utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet patients' health needs. Strong communication skills and effective use of available resources will be essential to promote quality care. The Care Manager will focus on meeting patient needs, managing length of stay, and promoting efficient utilization of resources. Determining medical necessity and appropriateness of admission will involve the use of screening criteria. The role requires a professional who is dedicated to improving patient outcomes through effective care management practices.
