Position Title: Title IIIB/Outreach Program Coordinator
Division: Aging and Disability Services
Work Location: Florence, KY
Exempt Status: Contract - Hourly non-exempt
Supervisor: Aging and Disability Service Associate Director
Supervises: N/A
This position is a contract position that will invoice NKADD for hours worked. The hours expected are full-time through with the end of the contract on June 30, 2027. As a contract worker, there are no benefit coverage or taxes deducted.
Position Purpose
This position is responsible for the coordination, oversight, and monitoring of Older Americans Act (OAA), Title IIIB Supportive Services within the NKADD region. The role ensures compliance with federal OAA requirements and Kentucky Department for Aging and Independent Living (DAIL) standards, while advancing Aging Area Plan goals related to access to services, community awareness, equitable service delivery, and the strengthening of community-based supports for older adults.
A key component of this position includes leading and implementing community-based outreach activities designed to increase awareness of available programs and services, expand access to underserved and priority populations, and strengthen the presence and impact of the Area Agency on Aging throughout the region. Outreach efforts include, but are not limited to, conducting presentations, participating in community meetings, and developing strategic partnerships with community organizations.
Responsibilities/Duties:
Essential - Title IIIB Supportive Services (Senior Centers):
- Monitor and evaluate senior center operations funded under Title IIIB and Nutrition Providers under Title IIIC to ensure compliance with OAA and DAIL requirements.
Ensure delivery of required supportive services, including:
- Recreation and social engagement opportunities.
- Outreach to older adults with greatest social and economic need.
- Education, including required monthly nutrition education.
- Information and Assistance services, including proper documentation.
- Health promotion and disease prevention activities (in coordination with Title IIID where applicable).
- Provide technical assistance to senior center managers to improve service delivery, compliance, and program quality.
- Facilitate quarterly provider meetings to support training, dissemination of policy updates, and peer learning.
- Support development and implementation of person-centered, community-based programming that promotes independence and reduces social isolation.
- Review and support provider reporting, including OAPPS data elements and performance measures.
- Assist providers in meeting requirements related to targeting priority populations as defined by the OAA.
Essential - Title IIIB Outreach & Community Engagement:
- Plan, implement, and evaluate outreach strategies that align with NKADD and Aging Area Plan goals, including increasing awareness of services and expanding access to underserved populations.
- Conduct targeted outreach to older adults with greatest social and economic need, including rural residents, low-income individuals, minorities, individuals with limited English proficiency, and those at risk of social isolation.
- Develop and deliver presentations to community groups, local governments, and partner organizations to increase awareness of aging services and support.
- Attend and actively participate in community meetings, coalitions, and events to promote the Aging Network and identify emerging community needs.
- Establish, strengthen, and maintain partnerships with key stakeholders, including healthcare providers, local government, community-based organizations, and social service agencies to expand service reach and coordination.
- Support implementation of public awareness initiatives consistent with NKADD and Area Plan goals, including regional campaigns and outreach efforts.
- Collaborate with NKADD programs (e.g., ADRC, Ombudsman, Waiver, Nutrition) to ensure coordinated, no-wrong-door access to services.
- Assist in identifying service gaps and barriers through outreach efforts and provide input and assist with planning, programs development, and continuous quality improvement.
- Develop and distribute outreach and educational materials to support informed decision-making by older adults and caregivers.
- Track, document, and report outreach activities, outputs, and outcomes in alignment with DAIL requirements and Area Plan performance measures.
Program Coordination & Compliance (Title IIIB):
- Ensure all services align with the Older Americans Act goals or promoting independence, dignity, and community-based living.
- Support implementation, monitoring, and evaluation of FY27-FY29 Aging Area Plan goals and objectives.
- Maintain compliance with all federal, state, and local regulations, including DAIL policies and procedures.
- Prepare reports, documentation, and monitoring tools to support oversight, accountability, and performance tracking.
- Collaborate with internal staff, providers, and community partners to coordinate services and maximize resources.
- Contribute to continuous quality improvement efforts and data-informed decision making.

Desired Minimum Qualifications:
- Bachelor's degree in human services, public administration, social work, or related field preferred.
- Experience in aging services, community outreach, or program administration is preferred.
- Experience with federal or state grant programs (OAA preferred).
Knowledge, Skills, and Abilities:
- Knowledge of the Older Americans Act, specifically Title IIIB Supportive Services.
- Knowledge of community outreach, public awareness strategies, and service delivery with the Aging Network.
- Ability to interpret and apply federal, state, and local regulations and policies.
- Ability to provide technical assistance and program oversight.
- Effective communication and public presentation skills.
- Ability to build and sustain community partnerships.
- Ability to analyze data and use it to inform outreach and program improvements.
- Ability to prepare clear, accurate reports and documentation.
- Ability to manage multiple priorities and meet deadlines.
- Proficiency in computer systems and data tracking/reporting tools.
- Ability to make sound, independent decisions.
Work Requirements:
Work is typically performed in an office setting with intermittent standing, sitting, or stooping. Position requires regular use of computer, phone, and standard office equipment. Travel within the service region is required for monitoring, outreach, and community engagement activities.
