Even though the role is remote, the candidate needs to live within the 5 boroughs: Manhattan, Brooklyn, Queens, The Bronx, and Staten Island. Type: Summer Internship Compensation: $20/hour, full‑time (40 hours/week) About the Opportunity We're looking for a Patient Navigator Intern to support our Health‑Related Social Needs (HRSN) care navigation program. Working alongside clinical and operations teams, you'll help patients overcome real‑world barriers – housing instability, food insecurity, transportation gaps – that stand between them and consistent, quality care. This role functions similarly to a case manager and is an ideal fit for someone pursuing a career in community mental health, social work, public health, or healthcare operations. What You’ll Do Screen patients for health‑related social needs : Conduct structured assessments using established screening tools and internal workflows. Connect patients to community resources : Help individuals navigate housing programs, food access, transportation support, and other essential services. Provide ongoing navigation support : Follow up with patients to ensure continuity and address emerging barriers as they arise. Collaborate across teams : Partner with clinical and operations staff to support individual care plans and document progress accurately. Support community outreach : Participate in engagement efforts to reach patients who may not yet be connected to services. What You’ll Learn Care coordination : Hands‑on case management experience with Medicaid populations. Social determinants of health : How unmet social needs directly shape clinical outcomes. NYC public systems : Familiarity with community resources, public programs, and local support networks. Behavioral health operations : Working within an integrated care model using remote tools and documentation workflows. Who we're looking for A current undergraduate or graduate student, or recent graduate, in social work, psychology, public health, healthcare administration, community health, or a related field. You bring: Genuine interest in community mental health, care navigation, or healthcare support Comfort discussing sensitive topics with empathy and professionalism Strong communication, interpersonal, and organizational skills Ability to work independently in a remote environment and stay accountable Openness to feedback, supervision, and mentorship Willingness to learn new systems and remote workflows quickly Internship Structure Fully remote internship One required in‑person training (approximately 3 hours) Full‑time, 40 hours per week for the duration of the summer internship Training provided on screening tools, navigation workflows, documentation processes, and remote systems Supervision and mentorship from experienced team members Why This Work Matters Many patients struggle not because of a lack of clinical care, but because of unmet social needs.
