NP/PA- Community Health Center

The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This position is a full-time, day-shift role, Monday through Friday, from 8:00 a.m. to 5:00 p.m. The provider will work collaboratively with physicians, nurses, and other healthcare professionals to deliver patient-centered care in a community health setting. Responsibilities include conducting comprehensive health assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans. The NP/PA will provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention. This role also involves managing follow-up care, coordinating referrals to specialists when necessary, and ensuring continuity of care for patients. The provider will maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives. Strong communication skills are essential for educating patients about their health, collaborating with interdisciplinary teams, and fostering trust within the community. The NP/PA will also play a key role in addressing social determinants of health by connecting patients to appropriate resources and support services. This position requires a commitment to compassionate, culturally competent care and the ability to work effectively in a fast-paced environment while prioritizing patient needs. This position is involved in a team-based approach to care.
