Registered Nurse - Rehabilitation

Job Description

Job Summary:

Provides inpatient rehabilitation nursing care to a diverse patient population, primarily focusing on stroke and neuro-related conditions. Delivers comprehensive assessments, plans, implements, and evaluates individual care plans while maintaining safety and quality standards.

Responsibilities:

• Assess patients' mental, physical, and spiritual needs to develop appropriate care plans.
• Implement and evaluate nursing interventions aligned with patient needs and goals.
• Educate patients and their families about care procedures and health management.
• Monitor and recognize changes in patient condition, reporting significant findings promptly.
• Maintain accurate, complete medical records in accordance with regulatory standards.
• Collaborate effectively with the healthcare team to ensure optimal patient outcomes.

Qualifications:

• Possess an active Illinois Registered Nurse license.
• Hold current CPR/BLS certification.
• Demonstrate experience in inpatient rehabilitation or neuro-specific care preferred.
• Exhibit strong clinical judgment and assessment skills.
• Communicate professionally with patients, families, and team members.
• Show commitment to continuing education and professional development.
