Family Nurse Practitioner, Mobile (BK)

Function:
We are seeking a dedicated Nurse Practitioner to become part of a collaborative, multidisciplinary healthcare practice serving the Brooklyn community. This position focuses on providing high-quality adult primary care, emphasizing disease prevention, long-term health management, and patient education within a diverse patient population.
Key Duties:
Deliver comprehensive outpatient medical care for adults, including wellness visits, health assessments, preventive screenings, and management of ongoing medical conditions.
Evaluate, diagnose, and treat a wide range of acute illnesses and chronic health concerns.
Work closely with physicians, advanced practice providers, nurses, behavioral health professionals, and other care team members to ensure coordinated treatment plans.
Educate patients on healthy lifestyle choices, preventive healthcare strategies, medication adherence, and self-management of chronic diseases.
Accurately document patient encounters, treatment plans, and clinical outcomes within the electronic medical record system.
Support organizational quality initiatives and actively participate in clinical meetings and performance improvement activities.
Perform other responsibilities and special projects as assigned by leadership.

Requirements:
Medical degree ( NP, or PA) from an accredited institution


Board certification


Current New York State license or eligibility for licensure


Experience in community health or outpatient primary care preferred


Excellent communication and interpersonal skills
