Job Summary The Comprehensive Prenatal Health Worker I (CPHW I) provides support services, ongoing orientations, assessments, care plan development, appropriate nutrition, health education and psychosocial interventions and referral when needed, to UCHC’s prenatal patients. Duties/Responsibilities Conduct face‑to‑face interviews with all prenatal patients, screen for presumptive eligibility (PE) and issue PE card if eligible; issue the 2nd good‑through extension for patients. Identify opportunities and facilitate efforts to expand service or increase patient access into programs. Assure client charts are documented whenever telephone & face‑to‑face contact is made. Code all super bills accordingly to type of visit and sign all chart entries using legal signature and title. Provide ongoing orientation, assessments, classes and make appropriate referrals when needed to prenatal patients. Prepare all intake packets in English/Spanish and ensure all appointments are given to patients accordingly (Intake, Initial, Medi‑Cal, etc.). Assist patients in completing Medi‑Cal forms (application) and follow up on no‑show/missed appointments. Order all prenatal forms as needed and fax weekly PE enrollment summary forms to the state. Fax weekly schedules to Medi‑Cal worker and advocate for patients with the Medi‑Cal worker. Case‑manage the prenatal program, reviewing all prenatal charts after each clinic visit with the provider and day before the visit to ensure all procedures, tests, etc. ordered have been completed and lab results received in a timely manner. Attend recommended job‑related trainings provided by the CPSP County program. Non‑Essential Functions Perform other related and/or necessary tasks as directed by the Associate Medical Director/Director of Women’s Health to achieve organizational and programmatic goals. Professional Requirements Adhere to dress code; appearance neat and clean. Maintain patient confidentiality at all times. Report to work on time and as scheduled. Maintain compliance with all state, federal and local regulations. Represent the organization positively and professionally at all times. Comply with all organizational policies and standards regarding ethical business practices. Communicate the mission, ethics and goals of the organization. Participate in performance improvement and continuous quality improvement activities. Have reliable transportation. Qualifications High school diploma or GED required. Medical Assistant (MA) certificate required. Current Basic Life Support certification. Zero (0) to two (2) years of experience in a community health center setting. Certification as a community health worker preferred. Comprehensive knowledge of prenatal care in outpatient settings required. Physical Requirements and Environmental Conditions Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions. Pushing and pulling objects up to 25 lbs. Frequent wrist, hand, and finger dexterity to perform fine motor function without tremor. Full range of body motion including twisting, pushing, and pulling. Light to moderate work with 25 lb maximum weight to lift and carry. Reaching, bending, stooping, handling objects with hands/fingers, talking/hearing, seeing. Work Schedule Generally scheduled Monday through Friday, starting between 8:00 a.m. and 9:00 a.m., ending between 5:00 p.m. and 6:00 p.m.; occasional Saturdays as needed.
